Credentials

The requested credentials (username and password) using the appropriate form will be provided directly to the recipient, upon presentation of the recipient's identification document. Alternatively, they may be delivered to another individual designated by the recipient, upon submission of a written proxy signed by the recipient, along with a copy of the recipient's identification document.
